Patient and Family Centered Care

Patient Family Centered CareWhat is Patient and Family Centered Care?

It is an approach to health care that:

Actively engages patients and families as partners to shape policies, programs, facility design, and day-to-day interactions.

Patient and Family Centered Care (PFCC) has been shown to lead to better health outcomes, wiser allocations of resources, and greater patient and family satisfaction.

It is a partnership based on equality

South Shore Hospital providers: Listen with care and seek to understand the perspectives and needs of patients and their families.

Patient and Family Advisors: Provide their perspective and ideas for improving the health care experience.

Core Principles of Patient and Family Centered Care

People are treated with respect and dignity.

Health care providers communicate and share complete and unbiased information with patients and families in ways that are affirming and useful.

Individuals and families build on their strengths through participation in experiences that enhance control and independence.

A collaboration among patients, families, and providers occurs in policy and program development and professional education, as well as in delivery of care.
